Two American Airlines jets nearly collide after sharing identical flight number
A pair of American Airlines planes had a close encounter in the US due to an error that assigned them the same flight number. The aircraft nearly met head-on because both flight crews believed the instructions from Air Traffic Control were meant for them. Once the controller realized the confusion, they started differentiating the flights by referring to them as "American 2482 on the arrival" and "American 2482 on the departure."
